WHO DO YOU SERVE?

We provide medication assistance and other client services to residents in the following zip codes in Yadkin County. Food assistance provided through the USDA Emergency Food Assistance Program (TEFAP) is available to eligible clients throughout NC.

HOW OFTEN CAN I COME?
Bread, produce, and TEFAP food is available to eligible households as supplies are available. Clients requesting medication and other client services may do so once per month.
WHAT DO I NEED TO BRING?
When you come to Yadkin Christian Ministries seeking medication and other client services make sure to bring:

State-issued photo identification showing your current address

Any other government-issued identity cards

A list of everyone in your household -- their names, date of birth, and relationship to you

The prescription, bill, statement or agreement for which you need assistance
ARE THERE ANY FORMS TO FILL OUT?
On your first visit to Yadkin Christian Ministries, you will need to complete our Client Information Form, if you are requesting medication and other client services. For your convenience, you can download a PDF version below and have it filled out when you arrive.
